Payment

 

Clients are responsible for the full fee at the time of service.  While I am not an in network provider with any insurances, I do offer courtesy billing to all insurances as an out of network provider.  Therapy is often covered in part or in full under mental health benefits.  It is highly encouraged that you call your insurance provider for pre-authorization in advance to determine how much they will cover.  The codes for Individual Therapy are 90834 or 90837 and the code for Family Therapy is 90847.

 

Important questions to ask include:

Are mental health benefits covered under my plan?

Do you require a referral from my primary care provider?

What percentage of each session will be covered for an out of network provider?

Have I met my deductible?

Are there visit limits to my mental health benefits?

Please be mindful that your contract is between you and your insurance provider.  I will submit the bill and your proof of payment.  Then, according to your benefits through your insurance provider, they will reimburse you directly.  Clients are responsible for full payment at the time of service regardless of insurances reimbursement rate.

24 hour cancellation notice is required or client will be billed for the session.
